Your Kaiser HMO Option

With your Microsoft Kaiser HMO Plan, you can expect a wide range of care and support to help you stay healthy, including no-cost preventive care, no-cost telehealth, and doctor’s office visits and prescriptions for a copay or coinsurance — even before you meet your deductible.

Explanation of Benefits are not a bill. They are a summary of services you’ve received. It shows the charges, the date of your visit, and the name of the provider you visited. Use them to:

  • Keep track of your expenses and make sure everything is accurate.
  • Check your progress; have you reached your deductible or out-of-pocket maximum?

Be sure to save your EOBs in case you need to validate a claim or for income tax purposes.

Your Kaiser HMO Plan

A set amount you pay for covered services. For example, when you check in at your doctor’s office, the receptionist might say you owe a $10 copay for that visit.

If the total costs you have paid for covered services—including copays, deductible, and coinsurance—add up to the out-of-pocket maximum then the plan will pay 100% of the costs for most covered services for the remainder of the year.

You have full access to a broad network of Kaiser Permanente and affiliate providers. For most specialties you can obtain a referral from your primary care physician by email, phone, video, or during an in-person visit. Emergency care is covered worldwide and is always considered in network. 

Out of network coverage is generally not included in your plan expect for emergencies and for urgent care when away from home. You are fully covered for emergency care and urgent care worldwide. 

Yes, your primary care physician will coordinate all your care and refer you to appropriate specialists if needed.  You cannot seek care from a specialist without a referral from your primary care physician. For most specialties you can obtain a referral from your primary care physician by email, phone, video, or during an in-person visit. Choose the right doctor

Start by browsing our online doctor profiles to find a doctor who’s right for you. You can search doctors accepting new patients by gender, location, and languages spoken – and view their photos, education, and credentials.

You can also choose a personal doctor with any of these specialties:

  • Adult medicine/internal medicine
  • Family medicine
  • Pediatrics/adolescent medicine (for children up to 18)
  • Obstetrics-gynecology

Each covered family member may choose his or her own personal doctor. And if you want to change to another Kaiser Permanente doctor at any time, for any reason, it’s easy to choose a new doctor online or by phone.

Before you enroll

If you enroll in the Kaiser HMO option, you'll need to receive all nonemergency services from Kaiser Permanente doctors, facilities, and affiliated providers as described in the Kaiser Permanente coverage documents when your coverage becomes effective. Except in certain circumstances when continuity of care requirements apply, you may not be able to access routine or ongoing care from your current providers.

Getting care

If you get hurt or sick while traveling, you’re covered for emergency and urgent care anywhere in the world.

Outside of Kaiser Permanente States, members can get urgent and emergency care through Cigna’s PPO Network* providers and urgent care at various MinuteClinic (in select CVS and Target stores), Concentra urgent care centers.**
