HMO, EPO, & POS 2-Tier Member Details for Cigna Healthcare Providers

Cigna Healthcare PPO Network providers outside of Kaiser Permanente states can easily identify eligible members by checking for a new Cigna Healthcare co-branded Physical ID card or Digital Membership Card, indicating plan participation. However, if a new card is not presented with a Cigna logo, Cigna Healthcare PPO Network providers are encouraged to call 888.831.0761 for verification of eligibility.

When our Kaiser Permanente members make an appointment, walk-in, or arrive by ambulance, they are to either:

  • Show their Digital ID card in the Kaiser Permanente mobile app. It has enhanced details to help the Cigna Healthcare PPO Network Provider verify information and where to send their claims. For eligible members, the back of their updated digital ID card says:

  • Show their new travel card. For eligible members, the back of their new Travel ID card says:

PPO & 3-Tier POS Member Details for Cigna Healthcare Providers

**Eligibility Exceptions: The Cigna Healthcare PPO Network* enhanced experience for our members is not available to:

  • HMO, EPO, PPO, & POS members enrolled in coverage issued by Kaiser Foundation Health Plan of Washington and Kaiser Foundation Health Plan of Washington Options, Inc.
  • PPO and POS 3-Tier members enrolled in coverage issued by Kaiser
    Foundation Health Plan of Northwest.
  • Medicare members
  • Medicaid members (Medi-Cal in California)
